Ascending aorta flow derangement is a marker of outflow obstruction in hypertrophic cardiomyopathy

چکیده
Background Alterations in systolic blood flow are a characteristic finding in patients with obstructive hypertrophic cardiomyopathy (HCM). Echocardiography is the gold standard for hemodynamic assessment in HCM, but this technique is generally limited to measurement of the left ventricular outflow tract (LVOT) pressure gradient and may not provide a complete description of the hemodynamic impact of the disease. In this study, we sought to employ time-resolved, three-dimensional phase contrast (4D flow) MRI to visualize and quantify 3D blood flow patterns in the LVOT and ascending aorta (AAo) in patients with obstructive and nonobstructive HCM.

Three-dimensional haemodynamics in patients with obstructive and non-obstructive hypertrophic cardiomyopathy assessed by cardiac magnetic resonance.
AIMS The left ventricular outflow tract (LVOT) peak pressure gradient is an important haemodynamic descriptor in patients with hypertrophic cardiomyopathy (HCM); however, secondary alterations in aortic blood flow have not been well described in these patients. Aortic flow derangement is not easily assessed by traditional imaging methods, but may provide unique characterization of this disease....
متن کاملIntraoperative Doppler echocardiography in hypertrophic cardiomyopathy: correlations with the obstructive gradient.
Although significant pressure gradients can be recorded across the left ventricular outflow tract in patients with hypertrophic cardiomyopathy, controversy exists regarding the presence or absence of true obstruction. Ten patients with hypertrophic cardiomyopathy were studied at the time of septal myectomy. A sterile continuous wave Doppler transducer was placed on the ascending aorta and direc...
متن کاملLeft ventricular outflow tract obstruction with systolic anterior motion of the mitral valve in patient with pericardial effusion caused by ascending aortic dissection -A case report-
Left ventricular outflow tract (LVOT) obstruction with systolic anterior motion (SAM) of mitral valve is not only limited to patients with hypertrophic cardiomyopathy. A diagnosis of LVOT obstruction with SAM is important because conventional inotropic support may potentially aggravate hemodynamic deterioration. We present a case of LVOT obstruction with SAM in a patient who underwent an emerge...
متن کاملPrevalence of aortic dilation in hypertrophic cardiomyopathy.
Recent studies have suggested that hypertrophic cardiomyopathy (HCM) is associated with increased stiffness of the aorta. However, a potential relationship between HCM and aortic dilation has not been established. Aorta size was characterized in 223 consecutive patients diagnosed with HCM. Aorta size was measured at the level of the sinuses (n = 223) and ascending aorta (n = 115) using the para...
متن کاملAdverse effect of nifedipine on left ventricular obstruction detected by pulsed Doppler echocardiography.
The effect of nifedipine on the ejection flow pattern in the left ventricular outflow tract was observed using pulsed Doppler echocardiography in 6 patients with hypertrophic cardiomyopathy. After sublingual administration of nifedipine (10 mg) to 1 patient, the turbulent ejection flow pattern became more marked and increased in duration compared with the initial state. An increase in turbulenc...
